Pharmacy Service

The CHAW PCN pharmacy team include prescribing pharmacists, pharmacists and pharmacy technicians. The pharmacy team help practices with a wide range of medicine related matters. This in turn, supports the health and wellbeing of the local population. The team work with a diverse group of healthcare professionals. They work in General Practice and in wider secondary care.  The team are currently focusing on: 

  • Structured Medication Reviews (SMRs) are appointments to review your medication. They usually last about 30 minutes.  They are over the phone or face to face. They check how you are doing with your medicines and if they work for you. The team will explain what each medicine is for. They will check to see if you need to change any medication. They may also determine if monitoring is necessary. You will have a chance to ask questions and any changes made will be in collaboration with you the patient.

  • Management of medicines at discharge from hospital. The team receive hospital discharge letters and enact any changes requested. This may mean a change to your prescription. This is so you have the right medication once you leave hospital.

  • Medicines safety. The team may change your medicines. This may happen because of product withdrawals or shortages, alerts, and guidelines. The team may recommend changes to medicines or dosages. This is if there are interactions or after pathology results.

  • Provide medicines information to practice staff and patients. The team provide telephone support for patients and staff. This could be general queries and concerns about medicines. This may also involve a follow up for patients to check the effect of any changes.

Suzanne Blythe is the Lead Clinical Pharmacist and is supported by:

  • 6 Clinical Pharmacists
  • 2 Pharmacy Technicians
