Mental Health and Wellbeing Service

If you are going through any social and / or mental health difficulties, your GP may refer you to our team. We have a range of professionals to work with you.

  • Care Co-ordinators often work with frail, elderly or vulnerable people. They often work with people with long term health conditions by providing support.

  • Social Prescribers often work with people suffering with many low level issues. These issues may stop you from living your daily life to the best of your ability. They will help you find strategies to improve your quality of life e.g. joining clubs, manage debt etc.

  • Mental Health Practitioners often work with people who have serious mental health conditions. They may assist with the creation of treatment plans. They may help with onward referrals (if needed) e.g. on to CMHT. They will work with you to find your needs and goals. They may offer a range of interventions. These interventions may include prescribing medication and support work. They may also include setting goals or helping to practice mindfulness. They may include skills-based work for mental health and problem solving. Referral into a Mental Health Practitioner is via your GP.
